Did you know? You could earn a HealthFund Healthy Actions credit* when you participate in a Condition Management program. Both enrolled employees and enrolled spouses are eligible. If your claims or Well-Being Assessment indicate that you may qualify, someone will call to invite you to participate. You can also request to participate. Just call 1-877-458-4975 (select option 3) and ask for a nurse to call you back.

The LM HealthWorks Plan has an enhanced Condition Management program, with a new name: In Touch Care (ITC).


The ITC program is designed to help members who have more serious health issues, like chronic conditions and acute medical events. For example, if you’re diagnosed with diabetes or high blood pressure or if you’re facing surgery or a hospital stay.

With ITC, you can connect by phone with a registered nurse, called a Care Manager, who can give you one-on-one support for high-risk health situations. You’ll work with the same Care Manager for all of your health issues. Your Care Manager can get to know you and your unique health issues, so you never have to repeat yourself. In fact, you can have the same Care Manager for all of your covered family members, too.

If your claims data or health assessment indicates you could benefit from an ITC Care Manager, an ITC representative will call to invite you to join the program. In addition, if you complete one goal (as determined in conjunction with your ITC Care Manager), you could earn a $100 HealthFund credit.

But, you don’t have to wait for a phone call. If you have been diagnosed with a condition, or have an upcoming surgery scheduled, just call Aetna Member Services at 877-458-4975, and select Option 5. Member Services can direct you to the appropriate Care Management program, to see if you qualify and/or to get started.
