How often will I receive calls from the care team when I participate in a Condition Management program?

Members will receive periodic calls from a care team member to support self-care efforts and physician's plan of care.

Member outreach varies for a number of reasons. The type and number of calls can vary by program type, acuity level, member's preference, events (hospitalization, urgent health changes) and to meet the member's health needs at a given time.

In light of all of these factors, member outreach is a fluid and variable component of the condition management programs. Telephonic outreach can vary based on the factors.
